Line Item Description

This NDIS support item provides a mechanism for providers to manage costs when a participant must immediately and permanently leave their Supported Independent Living (SIL) accommodation due to an irretrievable breakdown of the support arrangement.

Purpose and Application
This line item is intended for specific and critical situations where a participant's living arrangement is no longer tenable. It can only be used when:

There is an irretrievable breakdown of supports or relationships.

The situation requires an immediate exit from the accommodation.

The participant's personal health and safety, or the safety of others, is critically compromised.

How to Claim
Providers can claim for this support under the following framework:

Unit of Measure: Claims are made on a weekly basis.

Claimable Rate: The claim should be for the weekly rate specified in the participant's plan.

Duration: A provider can claim for a total of 28 days, which can be submitted as four separate weekly claims.

Key Conditions and Limitations
Before claiming, providers must ensure they meet all required conditions:

Prior Agreement: There must be a recorded agreement, preferably a Service Agreement, between the provider and participant (or their nominee). This agreement must detail the notice periods for any intention to claim for an unplanned exit under these specific circumstances.

Notice Periods: If a participant gives notice to leave and then exits earlier than planned, the provider can only claim for the remainder of that notice period.

Claiming Frequency: A provider is only permitted to make one claim (for the total 28-day period) for this support item per participant during a single plan period.

Claiming Method: Providers who use this weekly SIL exit item cannot use the hourly SIL support items in combination. A provider must use either the Weekly SIL support model or the Hourly SIL support model, but not both.
