Support Details

This support item introduces new options and alternative ways of living for people with disability. Individualised Living Options (ILOs) are packages of support built by holistically considering an individual's preferences, strengths, assets, support requirements, and informal or community supports. An ILO is focused on the delivery of support, not the accommodation itself, and is not determined by home ownership or eligibility for Specialist Disability Accommodation (SDA).

This specific support item, Exploration and Design, helps participants explore the Individualised Living Options paradigm and design an option that is suitable for them.

Providers delivering this support will assist the participant by:

Exploring the participant's vision for their future home and helping all involved understand the full range of options for community support.

Gaining an understanding of the participant's strengths, focusing on what they can do, and understanding how informal, formal, and community supports will play a role in their life to design the individual support package.

Identifying strategies to help the participant choose where to live, who to live with, and how they are supported.

Finding out what is possible, assessing risks within the context of the participant's preferred way of living, and exploring if other options might be more suitable.

Designing and making decisions with the participant and others about the options.

Depending on the circumstances, these tasks may also include assistance with locating suitable housing or linking the participant with other funded, community, or mainstream services.

This support item can be delivered to individual participants.

In addition to direct service provision, providers can claim using this line item for the following activities:
