This NDIS support item, 07_001_0106_8_3 Support Coordination Level 1: Support Connection, is a capacity-building support designed to assist participants in implementing their NDIS plan.

Purpose and Focus:

Support Connection is the foundational level of Support Coordination.

Its main goal is to strengthen a participant's ability to connect with and understand the broader systems of support, including informal, community, mainstream, and funded supports.

The service aims to build the participant's confidence and skills to understand their NDIS plan, connect with providers, and ultimately achieve effective utilisation of their plan.

It is a capacity-building support, helping participants take those first steps toward managing their supports with confidence and promoting long-term self-sufficiency and greater independence.

Key Activities and Components: Support Connection activities typically include, but are not limited to:

Helping the participant understand the details of their NDIS Plan and funded supports.

Connecting the participant with appropriate supports and services (NDIS providers, community, and mainstream).

Assisting in establishing supports and setting up service agreements.

Coaching, refining, and reflecting on support choices, and answering questions as they arise.

Providing assistance to connect with providers and maintain (or change) support relationships.

Reporting to the National Disability Insurance Agency (NDIA).
