This NDIS support item, 07_004_0132_8_3 Support Coordination Level 3: Specialist Support Coordination, is the highest level of capacity-building support, reserved for participants with highly complex needs or significant high-level risks in their situation.

Purpose and Focus:

Specialist Support Coordination (SSC) is an intensive, time-limited, and outcomes-focused service.

The primary focus is to reduce complexity in the participant's support environment and help them overcome immediate and/or significant barriers in plan implementation.

It is necessitated by complex barriers impacting a participant's ability to implement their plan, which may include interactions with the justice system, hospital/in-patient settings, or high-risk support environments.

The overall outcome is to ensure the participant receives tailored support, reduces risk, and increases their capacity for choice and control.

Key Activities and Components: SSC must be delivered by an appropriately qualified and experienced practitioner (e.g., Psychologist, Occupational Therapist, Social Worker, or Mental Health Nurse) who can provide the necessary expert and specialist approach.

Addressing Complex Barriers: Identifying, understanding, and establishing processes to overcome significant and immediate barriers that affect the participant's ability to access and maintain supports.

Design a Service Plan for Complex Support Needs: Developing a comprehensive service plan that ensures all services—funded, non-funded, and mainstream—work effectively and cohesively together. This is crucial for managing multi-disciplinary teams and complex mainstream interfaces.

Crisis Planning and Mitigation: Working with the participant and key stakeholders to resolve points of crisis, ensure consistent service delivery during unplanned events, and provide crisis planning, prevention, and mitigation strategies.

Negotiation and Coordination: Connecting, coordinating, and negotiating appropriate support solutions with multiple complex stakeholders to achieve well-coordinated plan implementation.
