NDIS Support Coordination Level 2: Coordination of Supports (Line Item 10_002_0106_8_3)
This support item is located within the Capacity Building budget of your NDIS plan. It is designed to strengthen your abilities to coordinate and implement the supports outlined in your plan, helping you to participate more fully in your community.
Level 2 Support Coordination, also known as Coordination of Supports, is suitable for participants who need hands-on guidance to navigate the NDIS and manage their services, but do not require the intensive support of a Specialist Support Coordinator. The ultimate goal is to empower you to take control of your life and build long-term resilience and independence.
A dedicated Support Coordinator will work with you by undertaking the following key functions:
Plan Implementation and Understanding: Breaking down the NDIS jargon and helping you fully understand the goals, funding, and supports included in your plan.
Service Connection and Coordination: Connecting you with a suitable mix of NDIS-funded providers, as well as community, mainstream, and other government services. Your coordinator will manage multiple providers to ensure all services work together effectively to meet your holistic needs.
Capacity Building: Developing your confidence and practical skills so you can independently manage your support arrangements over time. This includes assisting you to maintain relationships and efficiently manage service delivery tasks.
Monitoring and Review: Monitoring your plan budget and checking in to ensure that your supports remain relevant and effective as your needs change. They will also assist you in preparing for your NDIS plan reviews.
Crisis Support: Assisting with unexpected issues or challenges that may arise during your plan period, including planning and response to points of crisis.
This service is a crucial element in maximising your NDIS funding and ensuring the long-term sustainability of your support arrangements.
