It funds the provision of advice, assessment, and therapeutic intervention by an Exercise Physiologist (EP) to help participants manage the impact of their disability on their physical function, mobility, and overall health through exercise.

Key Outcomes and Activities:

Functional Assessment: The Exercise Physiologist conducts a thorough assessment of the participant’s physical capacity, including muscle strength, endurance, mobility, and gait.

Prescribed Exercise Programs: Develops and delivers safe, tailored, and evidence-based exercise programs to achieve specific therapeutic goals, such as:

Improving cardiovascular fitness and strength.

Increasing range of movement and flexibility.

Enhancing mobility and balance to reduce the risk of falls.

Managing pain and fatigue associated with chronic conditions or disability.

Capacity Building and Training: Provides training and education to the participant, their family, and support workers on how to correctly and safely implement the exercise program and integrate physical activity into their daily life.

Chronic Disease Management: The EP is specifically trained to help manage and prevent chronic diseases (e.g., diabetes, heart disease, obesity) through clinical exercise prescription in the context of a disability.

Professional Requirement: This support must be delivered by an Accredited Exercise Physiologist (AEP) who is accredited by Exercise and Sports Science Australia (ESSA).
