Line Item Description

This support item is located within the NDIS Capacity Building support category, under Increased Social and Community Participation (Category 13).

The purpose of this line item is to provide individualised support to assist a participant with a disability in making a successful transition through school and into further education or training. This is particularly relevant for students finishing school (Year 12) and planning their post-school life.

Key Outcomes and Activities:

Transition Planning: Providing time-limited, individualised support to develop a comprehensive plan for the transition from school to new environments such as university, Vocational Education and Training (VET), TAFE, or other education providers.

Skill Development for Study: Building the participant's skills necessary for success in further education, including time management, organisational skills, accessing and using learning resources, and self-advocacy within an educational setting.

Navigation and Access: Assisting the participant to research, visit, and apply for courses, liaise with educational institutions regarding necessary supports and reasonable adjustments, and navigate the application processes.

Non-Core Support: This item is intended for non-core supports that are outside of the educational curriculum itself (which is the responsibility of the education system). It focuses on building the participant’s capacity to engage in and manage their chosen educational pathway.

Liaison and Coordination: Working with school staff, disability support services at the tertiary institution, and other key stakeholders to ensure a seamless and coordinated transition.

Professional Requirement: This support is typically delivered by qualified professionals such as Mentors, Support Coordinators, Specialist Transition Support Providers, or other relevant professionals.
