The primary goal of this support is to facilitate functional improvement after maximum medical improvement has been reached. It aims to help participants with adjustment, adaptation, and building capacity for independence and community participation.

Services funded under this item include assessment, planning, and delivery of therapy supports. This item can also be used for the assessment, planning, and delivery of Disability-Related Health Supports where they relate to a participant's functional impairment.

Key Features and Uses:
- Delivery Setting: This support can be delivered to participants individually or in a group setting.
- Maintenance Care: This item can be used for maintenance therapy, defined as ongoing support to maintain a participant's current level of functioning, prevent functional decline, or achieve small incremental gains.

Provider Qualifications
Supports must be delivered by a qualified professional:
- Physiotherapist: Must have a current AHPRA (Australian Health Practitioner Regulation Agency) Registration as a Physiotherapist.
- Therapy Assistant: Can be delivered by a Level 1 or Level 2 Therapy Assistant operating under the delegation and supervision of a qualified Physiotherapist.

Claimable Activities Providers can claim for the following activities when delivering this support:
- Direct Service Provision: Face-to-face or telehealth support delivery.
- Non-Face-to-Face Support Provision
- Provider Travel
- NDIA Requested Reports
- Short Notice Cancellation (with 2 clear business days' notice)

Providers may also claim for Provider Travel – Non-Labour Costs using the dedicated support item (15_799_0128_1_3).

NB: There is not such a thing as NDIS rate that has to be paid (or ndis physiotherapy fees). The price given in NDIS Pricing Arrangements is the maximum that can be charged. However, this does not mean that you have to charge at this rate. In fact, it is usually wise to charge less - a lot of providers do this in order to get more work.
